Case Management is done by a team or set of trained case managers at mental health care clinics. These people have a community setup and get together to help a patient who has mental health problems and help them on the path to recovery. Rehabilitation centers, Mental Health Hospitals and Clinic help these patients get a cure or teach their family the care plan for these kind of patients.
One way case management helps to provide proper care is by noticing how each patient in a case is cured by the mental health care provided by the case manager. The case managers get together and discuss their plans for each patient and follow a particular healing therapy and analyze the progress of the patient after each course. This helps each case manager experiment on the behavioral patterns or moods that patient shows to accepting the treatment.
There would be different case managers for each and every type of mental health disease. For example, a drug addict would be assigned to a rehab case manager and trauma patient would be assigned to trauma specialist manager. This type of treatments would help to ensure that proper people are assigned to patient with different mental health conditions.
