The most common types of advance directives are the living will and the durable power of attorney for health care (sometimes known as the medical power of attorney).

The living will

The living will is a legal document used to state certain future health care decisions only when a person becomes unable to make the decisions and choices on their own. The living will is only used at the end of life if a person is terminally ill (can't be cured) or permanently unconscious. The living will describes the type of medical treatment the person would want or not want to receive in these situations. It can describe under what conditions an attempt to prolong life should be started or stopped. This applies to treatments including, but not limited to dialysis, tube feedings, or actual life support (such as the use of breathing machines).

Health care directive should be witnessed by two people who can attest to your mental capacity or signed before a notary public. A witness may not be any of the following: a health care provider, an employee of a health care provider or health care facility, or your agent designated in the directive.

Advance health care directives (also called advance directives) are a way for you to give consent for certain situations where you might want or not want treatment. They can also be used to appoint someone to make decisions for you if you can't do so yourself.

Advance Directives. -People have the right to make decisions regarding their health care. Advance directives are legal documents that allow people to state what medical treatments they want or do not want in the event that they are unable to make decisions or communicate because of severe illness or injury.

An advance directive is only used if you are in danger of dying and need certain emergency or special measures to keep you alive, but you are not able to make those decisions on your own. An advance directive allows you to make your wishes about medical treatment known.The Importance Of Advance Directives. Stress among patients and their loved ones often stems from disagreements regarding health care decision-making at the end of life, including the choice of when to use or stop use of life-sustaining medical procedures.

Nurses help to meet this responsibility by ensuring that a client's wishes, as expressed in an advance directive, are respected, and by continuing to provide care and support even when life-sustaining treatments are discontinued. care directives in a living will are protected from liability. Nursing responsibilities regarding advance directives include: Provide written information regarding advance directives. Ensure that the advance directives reflect the client's current decisions.
