Per reviewed journal on skin integrity exploring:

Skin is the largest organ in human body. Skin acts as a barrier to protect from infection. Skin integrity is defined as continuity of skin and it indicates healthiness of the skin. The contributing factors affecting skin integrity are moisture, friction rub with hard surface, prolonged pressure over the bony prominences and external trauma to the skin, fluid volume deficit in case of dehydration.

Peer review journal : Journal of wound ostomy and continence nursing:

The purpose of the study was to assess nurses knowledge on pressure sores and factors contributing to pressure ulcers. A cross sectional survey was done in Australian tertiary general hospital the data was collected among 2500 nurses working as full time and part time. 25% of samples was stratified by experience, seniority and graduation. The standard instrument Pieper-zulkowski pressure ulcer knowledge test was used to collect data. It consists of 72 statements with 3 sub scales . The tool consists of prevention of pressure ulcer, stages and wound description. The results of the study reveals 60% of nurses have inadequate knowledge on pressure injury and wound dressings. The recommendation of study is to initiate educational programs for nurses regarding pressure injury factors, prevention and standards in wound dressing.

Elderly people are at a greater risk for bowel and bladder dysfunction. Urinary incontinence affects 5% - 15% of elderly population. Bowel and bladder alteration affects elderly people emotionally and psychologically. This article states active nursing intervention helps to reduce burden. Nurses can initiate care by promoting physical exercise, pelvic floor muscle strengthening exercises, bladder training, usage of absorbents or diapers. To help to maintain hygiene.
