a)My first action would be checking for apical pulse for full one minute before administering the drug.

b)Assessment:

• Before administering the initial loading dose, have to make sure whether the patient has taken digoxin prior to this.
• monitor ECG throughout the IV administration and 6 hours after each dose. Notify the doctor if the patient develops bradycardia or tachycardia.
• monitor intake output chart, daily weight , assess for peripheral edema , auscultate lungs for crackles through the therapy.

c) i would administer 0.125 mg as in geriatric patients dose cannot exceed 0.125 mg.

d) Ms. penny 65 years old woman has congestive failure, cardiologist added digoxin 0.125 mg to her drug regimen. Her pulse was normal and she has not taken digoxin prior to this . 0.125 mg of digoxin is given and blood pressure and pulse was checked 30 min after giving the dose and they were normal.

e)shortness of breath might get reduced.
