His orders are for Percocet 1 tab for pain 3 to 6/10: Percocet 2 tabs PO for pain 7 to 10/10. c) What would be your first action? d) What dose would you administer and why? e) Thirty minutes after administering one dose, his pain is rated as a 7 on 0-10 scale. What do you do now? 1) Three hours after administering the medication dose, he rates his pain a 3 on 0-10 scale. Now what would you do? g) How would you document this episode?

a, Check patient pain score to calculate the pain level from low to severity.
b, patient blood pressure, pulse rate increased this indicate there is acute pain. his pain score 9 indicates the patient having severe pain. Ask the patient when did he consumed morphine dose and inform the doctor and get the order for pain relief.
c, Check the patient's last dose of morphine medicine.
d, If the patient was taken morphine sulfate within 4 hours administer the only the first dose of the Percocet 1 tab. If it is more than 4 hours then administer Percocet tablet 2.
