a)The first action here is making patient comfortable by making her to breath easily by administration of drug, which can decrease the workload of heart and thus prevent dyspnea.

Subjective data

  • Ask for any palpitations, allergic history ,urine output

Objective

  • Assess for heart rate before administration of pulse

c)The patient has to be administered Digoxin 0.125mg

d)The patient has been administered medication

e)Evaluation

  • The patient is able to breath easily
  • Able to do daily activities
