Ans) 19) Vancomycin 1000 mg oral every 6 hours.

Time: 6am- 12 pm- 6pm- 12 am

Available vial: 1 gm Vancomycin that

a. Dilute it with 10 ml normal saline, so that 1 ml = 10 mg

b. Concentration after reconstitution:

1 ml = 10 mg means 10 mL = 1000 mg i.e 1 gm.

c. As 1 gm = 1000 mg

So, 10 mL = 1000 mg, so 10 mL should be diluted further in 100 ml NS & should be administered through IV route.
