  1. Here heparin 25000 units dissolved in 250ml of D5

So, 25000units =250ml

1ml =25000/250=100units

Patient need 15ml/hr ie, 100×15=1500units

So, heparin 1500units is given per hour.

2. Recommended dose 0.5-2mg/kg/24hour

Here patient weight in kilograms is 43

If 0.5mg×43kg is 22mg and 2×43is 86mg

So, range is 22mg to 86mg is recommended however in this state should not exceed 80mg

So the patient take 1.75mg/kg ie, 75mg

  • Ordered dose is not safe because 45mg twice in a day means total patient required 90mg. (Mentioned not exceed 80mg)
  • Medication administered,  

Here 15mg=5ml

45mg=xml

Cross multiplication, 45×5/15=15ml
