Answer #1

1)5% dextrose water is isotonic solution and Dextrose water in half normal saline that is 0.45% is a hypotonic solution.because isotonic solution has the same concentration among the two solutes and solvent and hypotonic solution is the solution having sodium less than 0.9%.

2)Early enteral feeding in abdominal surgeries is always recommended which leads to quick healing but there are complications associated with it.Certain patients may not tolerate feeds until their GI functions are reversed as evidenced by presence of gas, flatulence.certain patients have vomiting and diarrhea due to poor absorption post surgery .This could be a rationale for continuing with fluids for 4 days .

3) Nutrition is an important aspect in better healing of patients so any patients hospitalized should be started on oral or enteral feeding unless contraindicated.a rules tube can be kept in the patient as long as possible so enteral feeding can be short term or long term therapy depending on the indication for feeding .post abdominal surgery the enteral feeding will be for a short term therapy till the patient regains Gastrointestinal functions like passing of motion, flatulence then slowly the rules tube will be removed and patient would be started on clear fluid orally then step up to complete diet .

4)Enteral formula feeds comes in different varieties like the elemental formula, polymeric formula, specialized formula.Ensure is the brand name of the formula by Abbott company ensure formula is a polymeric formula which means it has while proteins and complex carbohydrates which make it difficult to break down .Thus taking more of energy to digest so this type of formula is prescribed for individuals with complete gastrointestinal function .This therapy is given for a period of 6 weeks .The other forms of formula is the elemental formula which has proteins, carbohydrates, fats in their simple or elemental form thus making it easier to break down .This can be prescribed for patients with compromised gastrointestinal function like immediate post operative patients .
