Description of skill -

An apical heart rate or apical pulse is heard by a stethoscope on the chest. The site is the fifth intercostal space in adult and fourth intercostal space in the younger children and on the midclavicular line.

Tell your patient to breathe normally with the nose, it actually helps for better hearing of apical heart rate. because there may be excessive breath sound of the chest to disturb.

Count the lub and dub sound of heart for 15 seconds and multiply by four, along with this look for rhythm and type of sound.

Indication -

If there is any abnormality of heart like,

Tachycardia

Bradycardia

Arrhythmias

Heart disease

No peripheral pulse

Outcome evaluation

Evaluate the rate of heart, an irregularity that is arrhythmias of the heart, and listen to an abnormality in the heart sound like a murmur, S3, and S4.

Education

It is a simple procedure only requires a stethoscope. so, there is no education required but you can educate the patient about there disease and its treatment.
