Answer: As soon as nurse get to know about the complaint of neurological deterioration from the patient, a quick action should be taken. As the neurological disorders is the symptom of any problems occuring in the central nervous system.

The nurse should intervene the conditions and give the baseline treatment. The Symptoms should be observed such as palpitations, level of consciousness, heart rate , temperature of the body etc. Because if the level of consciousness is becoming poor then the condition of the patient will become more serious because brain may stop working and it can become fatal. The body temperature also plays an important role to know the condition of the system as high temperature may cause discomfort to the patient.

Nurse should check the verbal response, level of unconsciousness, motor response. Sometimes many drugs such as benzodipine may affect the level of consciousness in the patient. The GCS assessment is done for better response. The nurse are the first who should properly intervene the conditions and report to the medical practitioner.
