- Mercy Medical Center is currently not meeting the national standards for heart failure or pneumonia readmission rates.

- The following policy procedure is an effort to decrease unnecessary hospital readmissions for Mercy Medical Center.

- Evidence suggests that the rate of avoidable hospital readmissions can be decreased by improving the discharge and transfer processes; improving communication and care coordination; and enhancing education and support for patients’ self-care.

- The SMART discharge protocol was developed to improve care for patients and to improve the discharge process.

- By implementing this policy, Mercy Medical will expectantly decrease avoidable readmissions.
