Case Study - Nutrition Tony is a 72-year-old man who received a new percutaneous endoscopic gastrostomy (PEG). Due to having a damage to his esophagus status post radiation from throat cancer and numerous bouts of aspiration pneumonia. He has been unable to maintain an adequate nutritional status for weight and nutrition and it was decided that supplemental enteral feedings would be beneficial. He is alert and oriented X 3 and lives with his 70- year-old wife (Cecilia), Tony will be receiving bolus feedings as well as two medications through the PEG. As the primary nurse you are responsible for development and implementation of a teaching plan for this family system. Tony and Cecilia both expressed to you that "I've never seen anything like this before. I hope I can do this right. They want to send us home soon and we just want to be ready. When the doctor was telling us everything, it seems hard to do but we want what is best for Tony. It is a relief in a way to finally have a way to help Tony feel better and gain some weight, he has lost 50 pounds and was so weak from not | being able to eat and maintain his weight.

a) Motivation- Motivation is an inner feeling that energize a person to work hard. By motivating the Tony and Cicilia they start to learn the procedure of feeding as early as possibl confidently. It will helps to learn the procedure very esily.

b) Readiness: It is important to be in a state of being fully prepared for feeding the patient before discharge. From the statement of Cicilia they want to be ready foe the procedure. By providing education about the necessity of feeding and medicine intake and proper education and practice, she will be get ready soon for PEG feeding.

c) Learning environment - During this time they will be under some sort of pressure while thinking about discharge and follow up in home, directly affect the learning ability. Nurse need to eradicate such type of mental stress and setting a proper learing environment to accept and learn the follow up properly.

d) Diagnosis - From diadnosis they come to know the PEG is necessary due to the esophagus status of post radiation from throat cancer and bouts of aspiration pneumaonia lead him in this condition. So this factors will affect the learnig ability, Nurse need to privide psychological support for learn easily.

2) During the teaching session Tony was an active learner because he he is well oriented and he can understand the situation very easily. It is important to explain the developmenal stages of diseases and possible outcome and educate him to modify his life style according to his condition. Provide psychological support and encourage him to take proper feeding along with medicine as per prescription.
