The patient-centered medical home is an accepted model for achieving the ACO goals.

Adoption of Accountable care organization model:

- The Affordable Care Act (ACA) adopted the accountable care organization (ACO) model, consisting of groups of providers and supplies of health care, health-related services, and others involved in caring for Medicare patients to voluntarily work together to coordinate care for the patients they serve under the original Medicare (not Medicare Advantage managed care) program.
