Epstein Barr virus is the most common agent for heterophile positive IM, usually seen in non immune young adults. after the invasion of virus, it is readily detectable in the buccal fluid even though symptoms are present or not. from time to time presence of antibodies can be seen in other body fluids including urine.

in the early stages of illness, the heterophile antibody test is somewhat insensitive, especially in the first week.in the third week of illness, the false-negative rate is reduced to 5 %.
