100 mg/250 ml is the concentration of bag. So, 0.4mg/ml is the concentration.

1 mg = 1000 mcg. 0.4 mg = 400 mcg. So, the concentration is 400 mcg/mL.

So, the volume to administer 225 mcg is 0.5625 ml/min.

So, per hour rate is 0.5625 x 60 = 33.75 ml/hour.

So IV pump should be set to infuse 34 ml/ hour.
