The drug designing generally follows a structure-based approach. All the drugs act by binding to some target in the body (can be a receptor, enzyme, non-receptor protein, or the other structure), designing the drug according to the 3D structure of the target binding is likely to give successful results (similar to lock and key mechanism). The drug can be an agonist or antagonist, but its structure must have access to binding to the target site. So, identifying the target is the first step in drug designing.
