Assessment involves thinking skills , data collection ( subjective or objective)
Diagnosis - important phase assist in planning and implementation of patient care
Planning - goals are decided and how they affect patient health and should be specific
Implementation - is phases in which goals are implemented
Evaluation - is phase in which outcomes are analysed and their effect on health.
