Is there any reason a living will/advance directive should not be followed for a person who cannot speak?

Living will and advance directive are legal and written instructions for their medical care. It guides the doctors and health care professionals when the patient is terminally ill and near the end of life. this end of life situation happens at any time. a living will that time explain about their wish by their own documenting about plan to avoid unnecessary suffering and when the patient is com, unconscious, in surgery, inability to communicate their wish will be changed due to poor communication, that time health care agent will communicate with doctors and make reasonable decsions for the patient behalf. A person who makes living will/advance directive must able to speaks to choose truly capable to deal with a stressful emergency situation. when the person can not able to speak they can not make the right decisions about their preference and willing it can be misunderstanding and it will be a stressful situation for everyone to make decsions.
