It is extremely unlikely that you would be able to transplant a kidney without someone in this class because you're not closely related to anybody in the group. However, even identical twins do not have identcal receptors or recognition proteins to one another. What causes this to happen?

Even identical twins do not have identical receptors or recognition proteins to one another because of the factor of HLA (HumanLeukocyte Antigens) which are the proteins located on the surface of the white blood cells and other tissues in the body, these are said to be a match.(i.e., their tissues are immunologically compatible with each other) responsible for the acceptance or rejection of the transplanted kidney.

There are three main types of HLA: A, B, and DR. There are many different specific types of HLA proteins within these three main types. The children can inherit a different combination of HLA from their parents, sometimes with an identical match of HLA in the case of identical twins or brothers or sisters. Besides this fact, there are many types of unidentified HLA involved in the recognition proteins to one another which would affect the kidney transplant though the main three HLA types match.
