Answer #1

Area of interest/situation: There is a breakdown of bolier feedwater pump (this pump is required in a power plant to feed water to the boiler which converts water into steam which further rotates the turbine coupled to a generator to produce electricity). Due to this breakdown, the spare pump is running and the existing pump is cordened off and needs immediate repair.

Interim plan/measure: The plant supervisor must address this issue immediately and create an interim plan to resolve this issue. He must send the maintenance people to the site to firstly identify the problem. The entire area must be cordened off and nobody apart from maintenance team should be allowed to enter. Pump supply must be cut off and pump repair checklist must be followed in order to identify the cause and mitigate the problem. After the problem has been identified, the team must then work on establishing timeline to mitigate the problem. The timeline must be shared with the plant manager. After agreement with plant manager, team must start the maintenance work and all the tools required for maintenance should be available beforehand. After maintenance, pump must be operated on manual mode to see if it works fine. After proper trial run, pump must be switched back to automatic mode.

2. Preventive plan/measure: In order to avoid such a situation of pump failure at critical juncture, the maintenance team along with plant head must properly devise a preventive plan/measure to overcome such a situation. Preventive plan must include the periodic maintenance schedule of the pump. It must specify the duration after which different checks must be carried out for the pump. The repair schedule must also be captured. Spares and consumables of the pump must be identified and replacement schedule of all the consumables must be defined and properly tracked by the plant head. Procurement team must work in unison with the plant team and spares and consumables must be ordered as per the schedule specified by the maintenance incharge. Procurement team must order based on the lead time of the items. Preventive plan must be developed in order to avoid emergency situations when critical equipment functionality is required.
