Hannah is a 14 year old who presents to the clinic with fatigue and complaints of getting really tired on her runs. She is a runner and is on the school cross country and track team. She has been previously healthy and is a vegetarian. Her last menstrual period was 2 weeks prior to the visit.  She states that her menses are every 4 weeks, and reports that they are heavy and last for 5 to 7 days. Hannah’s Hgb is 10.1 and her Hct is 31.2%. Her ferritin is 14.

1. What if any medications are indicated for Hannah?

2. What education should she receive regarding her medication and lifestyle?

1)

The main complaint of Hannah is heavy bleeding.

As hb is less than 12 gm % ; so she is anemic.

We give medication to treat anemia.

For treating anemia oral iron supplement should be given.

These includes pills, capsules, drops and extended release tablets.

Oral iron tablets include ferrous sulphate, ferrous gluconate.

Ferrous sulphate contains 100 mg elemental iron.

We also give tranexamic acid to prevent excessive blood loss.

NSAIDS can also given to reduce the amount of prostaglandins in the uterus lining.

2)

She should not take medication empty stomach. it can cause nausea and vomiting .

Avoid tea, coffee upto 1 hr after taking medication to prevent absorption .

Eat green leafy vegetables like spinach, bathua etc, jaggery, black gram, meat, liver, fish oil, sea food, nuts.
