SIGNIFICANCE AND IMPORTANCE OF:-

NURSE : A registered nurse is very important when it comes to giving care to al elderly patient as the nurse are the one who keeps the track of patient's medications and its timings. The nurse can give primary care to the patient during an emergency situation.

FAMILY MEMBERS : Family menbers are very important during the coordination of care of an elderly patient, as they can help the patient by just being there for the patient it will help them psychologically.  

CAREGIVERS : Caregivers are the one who gives basic care to the patient like maintaining hygiene, feeding, in other words assisting them in day to day life.

INTERDISCIPLINARY TEAMS : They are a group of people who reviews the work of all the individuals involved in the care of an elderly patient, they want to make sure if the patient's right has been violated or not, if all the ethics are being followed.
