multiply the height by wieht and divide into 3600
57kgX165cm/3600 = 2.61m2
physician order dose =50mg
50mg/m2 x 2.61m2 = 130.5mg(round to tens 131) per day
131mg/m2 divided by 2 = 65.5mg(round to tens 66mg BID.
