Social workers play an important role in hospitals, rehabilitation centers, community health agencies, mental health facilities, etc. They assure that the patient’s best interests are being met.

Some of the roles of a social worker in a hospital include:

• Provide supportive counselling to patients and families.

• Care planning and coordinating patient care.

• Assisting patients in creating advanced directives.

• Connecting services in the community.

• Educate patient about the illness and treatment options.

Apart from arranging for placement in nursing facility, Anita can also provide clinical and case management services to the patient, can also assist in providing financial help, counsel the patient regarding the illness, and make the patient aware about government assistance and benefits program.
