Answer A) Patient is involved in the informatics experience.

B) Circumstances- To make the informed decision about their medical condition with respect to all the data stored.

C) It was a positive experience where the medical history of the patients are stored with the help of informatics techniques. This helps to educate the patient with their own health conditions and also the hospital can acquire the medical history of the patient and provide essential care. This minimises the error with the access to the electronic health records. It also helps in increasing the patient commitment and engagement. Measures that were in place to ultimately cause it to be positive is that all the health records of the patient are stored electronically in one place and is accessible to both the patient and doctors to make informed decisions.

D) Negative experience is that the medical information of the patients that is stored electronically is prone to hacking. The measures to avoid such incidence is to protect the data with stronger encryptions and avoid any loopholes in the server.
