Describe a time where you were part of a patient’s care (student clinical or at work) and there was a breakdown in communication. This could have been between you and the patient, the physician, the advanced practice provider, the patient care assistant or any other team member.

Describe this scenario, how was the patient affected and what was the resolution?

* When I worked in the medical ward one of my assigned patient was came from Nepal admitted

   for treatment of chickengunya so i started to take assessment through asking questions regarding

   his present complaints,past history and family history of infection for preparing care plan.but that

   time i understand that he only knows his native language and not respond to my assessment

   questions.

* This language barrier between me and patient result in delay in doing nursing interventions to

   to his present condition.

* Finally I just called his family member who taking care of him in the hospital she knows english

very well that help me to overcome this situvation and give proper care to the patient.
