DEFINITION

Insertion of a small-bore tube into the stomach through the nasopharynx.

PURPOSES

  • Decompression of stomach.
  • To prevent or relieve nausea and vomiting after surgery or traumatic events by decompressing the stomach.
  • To determine the amount of pressure and motor activity of gastrointestinal tract.
  • To give gastric lavage.
  • To obtain specimen for laboratory studies.
  • To administer medication.
  • To meet the nutritional needs of the patient.

INDICATIONS

  • When the patient is unable to ingest, chew or swallow food but is still able to digest and absorb nutrients.
  • When the patient is unable to retain the food.
  • For the patient who refuses food.
  • Surgery of mouth ,throat and esophagus.

STEPS OF PROCEDURE

  • Identify the patient.
  • Check the physicians orders for any precautions.
  • Ascertain the level of consciousnesss .
  • Wash hands and put sterile gloves.
  • Explain the procedure to the patient.
  • Place the patient in high-Fowlers position.
  • Place mackintosh and towel across the chest.
  • Measure the length of the tube.
  • Cut the adhesive tape 10cm long and keep it ready to fix the tube.
  • Put on clean gloves.
  • Lubricate the tip of the tube about 6-8 inches with water soluble lubricant.
  • Insert the tube through the nostril to the back of the throat ,aiming back and down toward the ear.
  • Flex the patients head toward the chest after the tube has passed the nasopharynx.
  • Encourage patient to swallow by giving sips of water when possible.
  • Advance 3-4 inches each time patient swallows until desired length has been passed.
  • Do not force the tube.
  • Aspirate gastric contents and check pH using litmus paper if available.
  • Place the end of the tube in a bowl of water to check for continious air bubbles in water .it indicates wrong placement.
  • Ask the patient to speak.
  • Secure the tube with tape.
  • Make patient comfortable in bed and provide oral hygiene every 4-6 hours.
  • Discard waste and remove gloves and wash hands.
  • Record type of tube placed,aspirated returned and patient tolerance.

if the residual fluid aspirated is in normal limit,then return the gastric content to the stomach to prevent electrolyte imbalance and provide oral hygiene every 4-6 hours.
