Description of IV insertion and fluid initiation.
* It is process of puncturing a vein with a needle and administration
of fluid into the blood stream through a intravenous catheter.
Indication of IV insertion and fluid initiation.
* It help to administer fluid and electrolytes to maintain fluid electrolyte
balance.
* For providing glucose which is necessary for metabolism.
* To give vitamins and minerals.
* For emergency fluid replacement in life threatning conditions.
* To administer emergency drugs and blood products.
Nursing interventions.
* Pre procedure.
* Check physicians order for IV insertion and fluid initiation and
explain procedure to the patient.
* Washing hands before selection of puncture site for IV insertion.
* Looking for veins that are comparatively straight with enough length
to insert IV cannula. ( common sites are inner arm,dorsal surface
of hand and dorsal surface of foot.)
* Apply tourniquiet 6 to 8 inches above the veinpuncture site and promote
patient to clench the fist to dilate vein for make insertion of needle easy.
* Wear gloves and clean the selected venipuncture site using aseptic swab
from center to outward in a circular motion to reduce microorganisma on the
insertion site.
* Intra procedure.
* Pull the skin taut below the entry site using non dominant hand to stabilize vein
for needle entry.
* Place catheter 15 to 30 degree angle with bevel up and insert catheter through
into the vein in one thrust.
* When blood is seen in the lumen and lack of resistance is felt then reduce the
angle of catheter and position parallel to the skin and advance the catheter and
needle to 2cm after loosen tourniquet.
* Remove the needle from inside of the angiocath completely and attach IV tube
after flush with sterile solution as required.
* Use adhesive tapes to tap the cathetet by place one strip with sticky part under
catheter hub then the second strip sticky part side down over catheter hub and
the third strip sticky part side down over catheter hub.
* Put label on the venipuncture site as per hospitals policy.
* Post procedure.
* Remove gloves and wash hands to reduce risk of infection.
* Discard all soiled articles as per agencies policy.
* Documentation of procedure in the nurses record.
Outcome / evaluation for IV insertion and fluid initiation.
* Evaluate the patency of IV cannula by observe the
IV infusion site and infusion flow.
* Evaluate for swellig,colour change,inflammation and
leakage from the insertion site.
Client education of IV insertion and fluid initiation.
* Instruct the patient to keep the Iv inserted hand steady during
administration of IV fluids to maintain proper flow of solution.
* Educate the patient about signs of misplacement of IV cannula
like swelling,redness,pain during IV flow and sudden changes in
flow rate.
* Instruct patient to inform health care provider if any changes happen
in the insertion site and flow rate.
