I am not good writing. Weekly Clinical Journal, I do not know how to write, I just few down some information, Can you help me rewriting it Bold is the question and another is my answer. Please help me to rewriting it

SMART Learning Goal for this Week:

I was learning this week about how to do head and toe assessment. I learned how to make therapeutic relationship with patient, and about the different phases of therapeutic relationship such as orientation, working, and termination.

Also, I learn how to assessment to patient skin and all type of wound. Also, I learn more about all stage of wound and how to treatment and intervention.

Evaluation of Learning Goal:

The week was both productive and informative. The clinical learning sessions reflected real life health care environments which provided a great opportunity to put what we have learnt in classroom into practice.

SMART Learning Goal for Next Week: is to learn how to use nursing process in the patient care. The different elements of the nursing process. I want to learn more how the nurse do in the filed. I plan to do more assessment with the patient, practice more and want to be more comfortable when I am in patient so the patient will more understand what I want to do

Things that went well:

I think application of the skills learnt from the text book went well in terms of engaging in hands-on clinical experiences. The Clinical experiences ,as I was posted to see the patient and learn how to head and toe with the patient .

Things that could have gone better:

I could be more comfortable when I assessment patient.

Things I learned that didn’t come from a textbook:

One thing I learnt that is not in the textbook is just how clinical experiences can help stimulate ones critical thinking in solving patient’s problems. I observe a different type of edema

Smart Learning Goal for this week :

• To learn about head to toe assessment
• To maintain therapeutic relationship with patient and learn about different phases of therapeutic relationship
• To assess the patient skin and evaluate types of wound,and to get to See the treatment and intervention based on the wound type

Evaluation of this week goal :

The learning objectives of the week was met which was both productive and informative.The clinical learning session was reflecting the real life health environment which provided opportunity to explore the thing what we learned in classroom into clinical practice.

Smart Learning Goal for the next week :

• To learn about nursing process and different elements involved in nursing process
• To learn about nursing documentation
• To plan for assessment ,demonstrate good assessment by repeated practice and understand the patient needs based on assessment.

Things that went well :

• Application of skills learnt from textbook was practiced well in clinical practice.
• Demonstration of good head to toe assessment.

Things that need modification:

• To maintain comfortable nurse patient relationship.

Things that I learned in Clinical Practice:

Clinical experience inculcates the students  critical thinking thereby tge student gains more knowledge and skills and there by works smartly to the situation.

To do expert practice it not only involes past experience but at the same time the involvement of one towards the subject,the daily learning and passion what they carry to imorove oneself in the subject matter.
