Hello, why do we do a review of symptoms and a head to toe assessment for a client on a nursing H&P. Also please help with 3 differentil diagnoses for a young child with an acute cough x 5 days sore throat and some ear pain.

I think viral Bronchitis is 1. Please help with more :)

The review of symptoms and a head to toe assessment for a client on a nursing history and physical examination is to know the history of client and identify any changes in systems which are causing the present symptoms ( problem pertinent Review of systems ) or whether the symptoms involving one to two systems or complete systems.

Acute cough is due to viral bronchitis.

Sore throat is due to viral pharyngitis or tonsillitis. Which became persistent due to cold.

Ear pain or otalgia or ear infection is common in children, due to common cold or upper respiratory tract infection.
