The creatinine clearance of the patient is one-third of normal and Gentamicin is entirely eliminated by glomerular filtration. So, a daily dose of 100 mg will also be calculated as one-third.

Therefore, 100 mg * 1/3 = 33mg

So, initial dosage regimen for this patient would be 33 mg every 8 hours.
