Q.11. Ordered Rocephin= 250 mg IM Q 12 hours

Q 12 hours means every 12 hours

Available vial of Rocephin=1 g per 4 ml

=1000mg per 4 ml=250 mg per ml

Recommended dose for children= 50-70 mg/kg/day in two equal divided dose

Weight of the child=45 pounds= 45/2.2= 20.45 kg ( as 1 kg= 2.2 ponds)

= 20 kg as given that round of the weight in kg in nearest 10th place

As per weight of the child safe dose range = 50X 20 mg / day to 70 X 20 mg/ day

= 1000 mg/ day to 1400 mg/ day

Q.12. Ordered dose= 250 mg IM every 12 hours

Per day dose will be = 500 mg

This is between the safe dose range and hence is safe

Per dose required ml will be = 1 ml

As available is = 250 mg per ml

And required each dose = 250 mg

Hence 1 ml of Rocephin is required in ml in each dose.
