Patient-centered care:

Describe strategies to empower patients or families in all aspects of the health care process

Knowledge:

To empower the patients and families in all aspects of health care by involving them in the treatment plan.

Skills:

Engage the patient in self-care activities and enable the family members to promote health, safety, and well-being for the patient.

Attitudes:

Respect the patient's rights and values in the planning of care and maintain records of patient health condition.

Patient-centered care 0f 80-year-old male with multiple health issues.

Knowledge:

Assess the patient's conditions and needs of treatment by focusing on primary care management. Involve the family members while assessing patient condition and planning of care. Design plan of care according to his needs.

Skills:

Follow evidence-based practice for the benefit of the patient and involve the family members in managing and controlling of chronic diseases and thereby improve the quality of life. Accept the patient preferences in treatment choices and remove the barriers and challenges in the treatment process. Encourage the patient to involve in self-care activities and make the family members assist in the day to day activities.

Attitude:

Able to take good decision in the treating of older adults. The treatment should improve the person rather than worsens the condition. Involve patient and family members in the decision-making process to avoid risk and harm. Patient culture and values should be respected in caring for the patient. Educate the family members about the caring of older adults at home.
