care to improve patient care and provide experience. Primary care
medical home improves health care as patient-centered care in
medical homes it ensures the patient receiving necessary care in a
manner. Greenhouse project in traditional based caring homes
provides meaningful lives for elders.
These all the factors for nurses role in shaping their competencies
towards knowledge, skills, and attitudes to focus on patient needs
as patient-centered care with quality needs. It provides the
pathway for an affordable health care system and creates
interprofessional collaboration among nurses. in various settings,
it helps to provide services that traditionally help to solve the
problem. Acting as a navigator for patient family help finds
solutions for their needs.
