Admission of patient

Hospital setting: Patient who need a specific period of care by healthcare professionals for acute or chronic conditions.

Some of the questions which can be asked are

  • The major complaints which disrupts their daily activity
  • Complete physical examination
  • Personal history,past medical or surgical history

On observations :

  • Vitals changes
  • Alteration in airway ,breathing, circulation, loss of consciousness, confusion,acute exacerbating pain,any bleeding etc.,

Clinic: Patient who needs temporary care and are stable after treatment and rest in their home

Some of the question to be asked are

  • Complaints of the patient
  • The period of the symptoms

Observations

  • Differentiate whether it is a minor or major case,if minor can be treated if major can be adviced to visit a hospital

Emergency department :Patients who needs immediatecare to save lives or prevent cimplications

Some of the questions to be asked are

  • If patient conscious the cause of the problem to be asked, if unconscious collect information from the relative

Observations

  • No airway, breathing, circulation, loss of consciousness, profuse bleeding, abnormal vitals
  • Any open injury ,

Rehabilitation or Long term care:Patient who needs a care to recover to their normal functions for a prolonged period of time in months

The observations to be made are

  • The nature of physical or mental disabilities

Home care :Patient are cared by the healthcare professionalsin their residence

The question to be asked are

  • The medical conditions of the patient
  • The type of care needed for the patients

Observations to be made are

  • Physical and mental status
  • Skin conditions for any ulcers,musculoskeletal skeletal disorder as a whole

When a patient is to be discharged from a healthcare setting The following health concepts to be concerned

  • The health status of the patient
  • Educating on the use of medication and its side effects
  • Report in case of any abnormal symptoms
  • Encourage to visit for follow up to assess the progress,prognosis of the disease condition.
