Demings 14 point is a model for continuous quality improvement it lays emphasis that a quality improvement can occur only with process changes because when a process is changed there are less room or opportunities for errors so it's not the event which is improved but the entire process leading to the event is improved .its 14 points revolves around having a long term goal and a continuous or constant quality improvement efforts .it encourages reporting of errors without any punishment to understand the corrective and preventive action of the error which will further reduce such errors .
Model for improvement asks three questions which helps in the formation of mission statements.
What are we trying to accomplish
How will we know change is an improvement
What changes will we do to accomplish the improvement
So the first question helps us to understand what are we striving to achieve or what is our goal which will help us to make the first statement of patient safety mission statement.
The second question is about ways to measure or analyse the changes to understand whether it is really an improvement.
The third question tells about the changes that has to be implemented to bring about a improvement.
Example of a patient safety mission statement.
The hospital aims to provide safe and quality care to aptients by reducing the no of falls in elderly patients by 50% from the current practice by providing safe environment,regular audits and interdepartmental help.
This mission statement covers the first question the aim to reduce the no of falls .the measurement on no of falls by quality indicators and by change in environmental aspects of dry floors,anti skid floors .
