Description: Nasal packing is a treatment to control anterior and posterior epistaxis by using packing materials.

indications: anterior and posterior epistaxis

outcome:

  • bleeding will be stopped
  • healing will occur

potential complications:

  • Rhinosinusitis
  • toxic shock syndrome
  • aspiration
  • asphyxiation

nursing interventions to prevent complications:

  • Instruct the patient to avoid exerting herself, forcefully blowing her nose, or bending over.
  • She should also avoid NSAIDs, alcoholic beverages, and smoking for 5 to 7 days.
  • water-soluble ointment should be applied to lips and nostrils of the patient while packing is in place and to use a cool-mist room humidifier.
  • Advise patient  to prevent constipation and straining, as that increases the risk of bleeding.

preoperative:

  • ensure the right patient
  • maintain privacy
  • Assemble all the articles
  • informed consent
  • explain the procedure
  • Ensure bedside suction is functioning properly.

intraoperative

  • relieve patients anxiety by explaining every step
  • Obtain vital signs and SpO2 level, and assess her breath sounds. Administer supplemental oxygen via facemask if needed. Continue to monitor vital signs closely.

postoperative

  • reassess the patient and provide oral care. Keep the patient's mouth moist while the packing is in place.
  • place the articles at proper place
  • make the patient comfortable.

Client education

  • Instruct the patient to report signs and symptoms
  • he should be advised to go for follow up with an ear, nose, and throat specialist as an outpatient.
  • While packing is in place, Instruct the patient to avoid exerting herself, forcefully blowing her nose, or bending over.
  • Patient should also avoid alcoholic beverages, and smoking for 5 to 7 days.
    Patient should apply water-soluble ointment to her lips and nostrils while packing is in place and to use a cool-mist room humidifier.
