This is Yu.Zhou. May anybody who is good at the Supervisory Management in Health Care Organization class help me with the assignment below. Thanks.

Management Survey Assignment: (CO5) COG5)

Students are required to create a survey that their potential employees might be asked to complete which would evaluate their management performance. Students may do this in a Word Document or utilize tools like Survey Monkey. The Survey should be 10-15 questions, usually multiple-choice type questions with space for comments. Students are welcomed to research surveys used by other organizations, but should be creative in their approach to their own survey. Results of the survey should ultimately be able to provide a manager with a picture of their strengths, weaknesses, opportunities to improve and issues that are important to their employees. Successful managers are those who listen to what their employees think of and need from their team members.

I would like to see a sample of multiple choice survey questions that their potential employees might be asked to complete which would evaluate their management performance in your replying.
