Care coordination is the organization consists of more than two people to serve quality patient care services facilitating appropriate delivery of health care to patient.

Health is always a big issue.If individual or the community is healthy, there is less health issues then it is good for nation. So to ensure this delivering quality care to people at right time is important. But in order to make the correct use of resources of helath care facilities man power is always needed. Care coordination is the process which brings all the speciality care to people together under one shed and coordinators supervise the care to achieve the patient care goal and family needs. So its importance are-

• meets up patient needs and family needs.
• achieve health care goal.
• make people more quick to access the facilities appropriately.
• saves money of people as there is always fair cost services.
• easy availability of good treatment.
• improves patient satisfaction as patients are sometimes referred without knowing why they are referred to higher centres, what to do next etc.
• less efficient care can be avoided as information are communicated well,no chance of missing the informations during referral.

Care coordination can be incorporated by opening services towards people for primary health care with adequate resources-

1. using electronic media to know the status of care coordination services.
2. finding out the accountable organizations for patient care servservices and their effectiveness.
3. For adult patients needs which are basically complex in nature try to serve best quality care in patient centred medical institutions.
4. spreading care to local neighborhood etc.
