There are two parts to this assignment. The first part is to create a family health tree/portrait. The second part is to identify and describe two local community-based organizations that address the health needs of families. You must complete both parts of the assignment.

Part 1: Family History

A family health history is a written record of a family’s health. The history contains information about a family’s medical conditions, where and how family members grew up, family members’ lifestyle habits (for example, whether anyone in the family has smoked), and family members’ activities to promote their own health. It’s like a family tree for health. You can look online for examples of family health trees/portraits to see how you can create your own. Your family tree/portrait can be presented in a table format.

The first step in creating a family health history is to talk to your blood relatives. The most helpful information comes from ‘first-degree’ relatives — parents, brothers and sisters, and children. Information from ‘second-degree’ relatives — nieces, nephews, half-brothers, half-sisters, grandparents, aunts, and uncles — as well as less close blood relatives can also be used.

Part 2: Knowledge of Community Health Organizations for Families

Locate and describe two community based organizations in your local community that deal with health issues affecting families. This section should beabout 3 pages long. It is your responsibility to identify two appropriate organizations in your local community.

Include the following information about each organization:

Name of the organization

Date established?

Mission or purpose of the organization?

Government agency, quasi-government agency/organization, voluntary agency?

Who is the current head of this organization?

What services does it provide to families?

How is it funded?

Mailing address, phone number and web site address

Comment about the design and layout of the organization’s website

Identify FIVE new things that you learned from the website of each organization

How was this information helpful to you?

Include any other comments or information you deem relevant
