           implementing the plan.

Implementation

         * In implementation phase application of interventions planned by the nurse for patient occur.

          * Accurate implementation of planned activities help to better outcomes in the patient.

Evaluvation

          * In this phase nurse evaluvate the effectiveness of care through feedbacks from the patient.

          * Evaluvation process help to know about changes needed in future during taking care of

        Patient problem: Refuse baths.(Bedridden patent)

       Assessment

                 * During assessment phase patient said that " I felt cold in early morning this staffs perform

                    sponge bath in that time so i can't allow it".

       Planning

                  * Plan for change timing of sponge bath to make patient comfortable for take bath.

                  * Ask patient about his willingness to take bath in new timing. (After 9.00am)

      Implementation

                  * Given sponge bath at 9.30am and patient co-operate in doing the procedure.
