Coding system:the clinical coding systems assign a distinct numeric value to medical diagnosis, procedures and surgerym, signs and symptoms of disease and illdefined conditions, poisoning and adverse effects of drugs , complications of surgery and medical care, the medical coding systems currently used in the united states are ICD-10-CM, ICD-10-PSC and HCPCS it is the intenational classification of disease , tenth revision, clinical modificaition, medical diagnosis codes are used by all health care settings including hospitals, physicians, nursinghomes, home health agencies and other providers , ICD-10-PCS it is the international classification of diseases, tenth revision, procedural classification system,HCPCS it is the healthcare outpatient procedures and physician services., the ICD-10-CM it is the expanded set of diagnosis code which has replaced the ICD-9-CM , this ICD-10 -CM is a 7 character, alphanumeric code and each code with a letter and that letter is followed by 2 numbers that first three characters of ICD-10-CM are the category and this describes general type of the injury or disoease the category is followed by a decimal point and then the subcategory and this is followed by up to 2 sybclassifications, which further explain the cause , manifestation, location, severity and type oof injury or disease the last character is extension, this ICD-10-CM is more support the reporting of much more specific and detailed information about risk, severity, complexity comorbidities and complications and other key parameters of patient evaluation . And the specificity refers to the ability of an expression to provide more details about the nature of an object, condtion or process , the combination codes that combine a number of granular concepts together to arrive at a more specific description , and the analysis of ICD-9 and ICD-10 cides demonstrates that in most instancces , ICD -9 is usually more granular than ICD -10 but often less specific. many providers are concerned that they will be required to use highly specific codes for conditions when they do not have sufficient information, and ICD-10 allows for more specific description of a number of conditions. The diagnosis must be coded to the absolute highest level for that code meaning the maximum number of digits for the code being used , the diagnosis code needs to be more specific, example of a dagnosi needing to be billed to higher leel of specificty : Diabetes :ti indicate diabetes the code 250.0 is used and the 5th digit is needed to specify the type of diabetes so diabetes mellitus tyoe 2 is 250.00 , diabetes mellitus type one ( juvenile type) is 250.01 , diabetes mellitus type 1 uncontrolled is 250.02 and so on , without the 5th digit the claim is lacking enough information to be processed.
