Hemophilia is a clotting disorder resulting from Deficiency of factors VIII and XI , proteins needed for clotting mechanism.

Mr X 36 years old came to Emergency room with complaints of nose bleeding and it persists for more than 20 minutes. He also reported symptoms of severe joint pain and bruises over the elbow.

1) Enquire patient if he got any injury or fall that results in accident? Patient denies any injury.

Sudden nose bleed with out any injury is primarily due to clotting deficiency.

2) Enquire patient regarding family history of clotting disorders .

Mr X states his mother will have bruised joints.

This indicates Hereditary transmission of disease as Hemophilia is mainly transmitted through defective X chromosome. It mainly affects males and defective mutated chromosomes transmitted from mother to son.

3) Diagnosis : Physical examination helps to rule out bleeding inside the joints. Blood studies to rule out clotting factors

4) Instruct patient to avoid external injury . Health workers must take precaution before pricking patient for intravenous and Intramuscular injection. Educate patient to report blood in stool or urine as indicates internal bleeding.

Advice patient to use soft bristles to prevent gum bleeding and to avoid sports that cause severe injury.
