Answer: Weight gain during the first trimester should be 0.5 to 2.0 kilograms or 1.1 to 4.4 pounds, weight gain in second trimester should be 0.2 to 0.5 kilograms (0.5-1.0 pound) per week and in the third trimester the weight gain should be steady and minimal with the same as in the second trimester.

The nurse should intervene the vital signs as well as the client should empty the bowel. After this process the nurse should administer pain medication to the client. It is important that the client is in a comfortable and relaxed position.
