Solution:

Flow rate in mL/hr is taken as Y

So the Y = Ordered Per Hour/ Have X Volume (mL)

At first convert the ordered time into patient required amount. That is 69 kg is the weight in kg and 4 mcg/kg/hr is a dosage in terms of kg.

Weight in Kg X Dosage per Kg = Y (Required Dosage)

69 kg X 4 mcg/kg/hr = 276 mcg/hr

Then Convert 276 mcg/hr to mg/hr.

mcg → mg → g → kg    ( ÷ by 1,000 )

276 mcg/hr ÷ 1,000 = 0.276 mg/hr

Therefore according to formula

0.276 mg/hr / 18.3 mg X 351 mL = 5.3 mL/hr
