ANSWER - 7) a) Assessment are important in each phase of nursing process.

9) Rationale for enteric coated medications;

  • it prevent its dissolution in the gastric environment
  • prevent the release of drug from the medication.
  • Avoid chemical incompatibility.
  • Mask the odour , taste or color of the drug.

10) ANAPHYLAXIS is the most serious allergic reaction.

Anaphylaxis is an acute allergic reaction to an antigen. Substances that causes anaphylaxis include food, medication , dyes, etc

SIGN AND SYMPTOMS;

In mild reaction:- symptoms begins with in 2 hours of exposure. symptoms are warm sensation, sweating,tingling sensation, pruritus, swelling around the eyes , sneezing.

In moderate reaction :- In addition to mild symptoms other symptoms include itching, warmth , flushing of skin , restlessness.

In Severe reaction :- sudden onset . symptoms include bronchial spasm, laryngeal edema, dyspnoea, cyanosis, hypotension, . Cardiac arrest and coma may occur.

TREATMENT ;

Medication : Epinephrine, antihistamines, corticosteroids

IV Fluids : Vasopressor agents and volume expanders to maintain blood pressure

  • corticosteroids and aminophylline to improve airway patency.

  • Potassium - normal value ; 3.5 - 5.5 mEq/L
  • Chloride - normal value ; 98 - 107 mEq/L
  • Bicarbonate - normal value ; 24 - 32 mEq/L
  • BUN (Blood Urea Nitrogen) - normal value ; 7 - 20 mg/dL or 2.5 - 8.0 mmol/ L
  • Creatinine - normal value ; 0.6 - 1.2 mg/dL or 60 - 125 umol/L
