Desire dose is 500 mg

1000 mg = 1 gm

500 mg = 0.5 gm vancomycin iv over 2 hour

Available dose is 0.5 gm in 250 ml.

That is to be infused in 2 hours that is 120 minute.

In 120 minute 250 ml

So in1 minute how much ml

1×250/120= 2.08= 2 ml per 1minute

So set the infusion pump 2 ml/ minute to deliver desire dose of drug.
