1) You are told at lunch that one of the nurses from your unit was overheard talking about a client over the phone with his wife.

2) Your charge nurse asks that you chart a treatment she did because she is too busy to do it.

3) A client's family member asks that you show him his mother's chart, specifically the physician's progress notes.

4) You are running very late and need to pick up your children from school. You have only a few minutes to do your charting. Because you were busy all shift, you have a whole shift's worth of charting to do.

5) A physician asks you to do a treatment right now at the bedside that you are not very familiar with. You tell her politely and she says, "Do it anyway".

1.I shall caution her about the safety and privacy of the patient which we should maintain in our profession.Any sensitive information or data about the patients should never be disclosed. Which is completely unethical.
