The care can be provided only by the Registered nurse. She is responsible for the delegation of the task assigned to her. As a caregiver, she assesses the patient, plans for the treatment, and implement the plan. She administers medication, makes a clinical decision, and provides client education. She will not assign these tasks to their Practised nurse or APs.

For e.g In ischemia, insufficient blood supply to the tissue results in decreased oxygen level and leads to cell damage.

Underlying Principles:

Nurses require specific factors for the delegated task such as

• Knowledge and skills necessary to provide care

e.g The cause for ischemia is due to blockage of an artery due to decreased oxygen supply to the tissues.

Nursing Intervention:

• WHO - RN has to provide care.
• WHEN - The care has to provide as soon as possible to meet the therapeutic goal.
• WHY - To alleviate the symptoms, restore the functions, and prevent the complication.
• HOW - Care is delivered through therapeutic intervention.

e.g: Proper lab investigations and the diagnostic procedure is done to make an accurate diagnosis of ischemia and followed by therapeutic intervention.
